E/M services: Documenting Emergency Room Services (B)

Incorrect coding of evaluation and management (E/M) service levels for emergency room (ER) visits is among the top contributors of the Medicare improper payment error rate. The ER E/M Current Procedural Terminology® (CPT®) codes are 99281-99285. The highest level of service (CPT® 99285) is statistically being over-utilized and among the top codes that contribute to the improper payment errors. Incorrect coding and insufficient documentation are the two main reasons for these errors.

During the webcast, we will answer questions you have regarding documentation of the medical record and address the prepayment editing that can potentially impact your claims processing for E/M services. This webcast is intended for any provider who bills ER services and their coding and billing staff.

This webcast will provide an overview of appropriate coding and documentation requirements of emergency room services by explaining:

  • Individual code levels
  • Documentation of medical necessity
  • Prepayment editing in place for ER services
  • Resources available to assist in further understanding the guidelines for these services
